What Services Do You Offer?
Our home care services in Santa Clara are focused on providing our patients with compassionate and effective personal and companion care. This includes assistance with activities of daily living such as:

- Medication reminders
All of our caregivers go through specialized training to ensure that they are able to handle a wide range of issues and help prevent potentially dangerous situations. We can provide specialized care for patients who need assistance with dementia, Alzheimer’s, Parkinson’s, heart failure, arthritis, stroke, diabetes, or arthritis. Our caregivers are able to assist you with normal household tasks as well as advanced services like meal planning, transfer assistance, or transportation.
Can I Change My Care Giver?
At Polaris Home Care, we are focused on making sure that you receive home care that specifically addresses each of your needs and desires. To ensure that you are completely satisfied, we work with you to select the caregiver that you feel the most comfortable with. If at any time you are not satisfied with the caregiver that is assisting you, we will work with you to immediately find a replacement.
Why should I choose an agency over an independent caregiver?
Polaris Home Care offers distinct advantages over hiring an independent caregiver. Our agency is licensed with the State of California, bonded, and fully insured if an incident ever does occur. With this comes frequent agency inspections and compliance with strict guidelines and procedures set by the Home Care Services Bureau. All of our caregivers go through stringent background checks with the FBI and DOJ, drug testing, and at least 3 reference checks to verify their experience and ability to work with others. We also require all of our caregivers to complete 5 hours of initial training as well as at least 5 additional hours of annual training with our agency to ensure that they are providing the best possible care for our clients. This includes specialized training to prepare them to be capable of effectively working with clients with any disability to promote a safe living environment.

We provide this by working with your physicians, nurses, care managers, and family to assemble a care plan that addresses all of your needs while building a safe living environment to prevent any further illness or injury. In addition, we have a Registered Nurse available to you as well as our caregivers at all times to provide you with skilled advice for any situation. Our caregivers also go through a rigorous training program which includes specialized training for specific illnesses to help them provide tailored care and to be able to identify possible issues before they occur to prevent illness or injury.
